What to Bring to Your Exam

• Your insurance card and photo identification
• Your completed paperwork (optional but recommended)
• A list of all medications that you are currently taking
• Glasses and/or sunglasses you usually wear, even if you typically wear contact lenses
• If you wear contact lenses, wear them to your exam and bring a written copy of your current prescription
• Please verify your insurance benefits with your insurance company before your appointment
